Unzip the Patch number to some temporary location like /tmp using oracle user

% cp p[patch_id]_[version]_[platform].zip /tmp
% cd /tmp
% unzip p[patch_id]_[version]_[platform].zip
% cd

Try applying the Patch with opatch under debug option as

% export OPATCH_DEBUG=TRUE
% opatch apply
